  5.              DIRECTIVE TO PHYSICIANS

  10. I, Kristine Church, being of sound mind, intentionally and
  11. voluntarily declare: 
  12.  
  13. 1. If at any time I should have an incurable injury, disease, or
  14. illness certified to be a terminal condition by two physicians,
  15. and where the application of life-sustaining procedures would
  16. serve only to artificially prolong the moment of my death and
  17. where my physician determines that my death is imminent whether or
  18. not life-sustaining procedures are utilized, I direct that such
  19. procedures be withheld or withdrawn, and that I be permitted to
  20. die naturally, 
  21.  
  22. 2. It is my intention that this directive shall be honored  by my
  23. family and attending physician as the final expression of my legal
  24. right to refuse medical or surgical treatment and accept the
  25. consequences from such refusal.

  27. 3. If I have been diagnosed as pregnant and that fact is known to
  28. my physician, this directive shall have no force or effect during
  29. the course of my pregnancy. I understand the full import of this
  30. directive and I am emotionally and mentally competent to execute
  31. it.
